ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM AND PIRACY WARNING


I HEREBY ASSUME ALL OF THE RISKS WHEN STAGING EITHER OF THE AUDIO FILES
ASSOCIATED WITH THE “SCARE YOUR FRIEND” DIGITAL DOWNLOADSPSYCHO CLAN is providing you with digital downloads of two different audiophiles that can be staged as
a “Haunted House” in the safety of your own home. Psycho Clan is also providing manuals of how they
staged them when they have in the past. Psycho Clan presented them safely for audiences to enjoy, and
always without incident. Staging these Haunted Houses outside of their suggested parameters is not
recommended. If you choose to do so, or if any harm comes to anyone while performing these Haunted
Houses whatsoever, you indemnify Psycho Clan, LLC, Timothy Haskell, Inc and all other entities
associated with any of their productions, or as a corporation past or present
I acknowledge that this Accident Waiver and Release of Liability Form will be used by the Producers of
the audiofiles in which I may participate and/or produce on my own, and that it will govern my actions and
responsibilities..


In consideration of allowing you to download the SCARE YOUR FRIEND audiophiles and permitting
them to be staged either how Psycho Clan intended, or whatever artistic changes are made outside of that,
to participate in this activity, I hereby take action for myself, my executors, administrators, heirs, next of
kin, successors, and assigns as follows:


(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability for my staging, mounting, or
participating in the Haunted Houses included in SCARE YOUR FRIEND, and assume any risks
associated with mounting and/ or participating in such an event, THE FOLLOWING ENTITIES OR
PERSONS: PSYCHO CLAN, LLC, I CAN’T SEE, INC, TIMOTHY HASKELL, INC and/or their
directors, officers, employees, volunteers, representatives, and agents, and the activity holders,
sponsors, and volunteers;


(B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons
mentioned in this paragraph from any and all liabilities or claims in the unlikely event of injury while
participating this activity.


I acknowledge that PSYCHO CLAN, LLC is providing me merely with audiophiles as well as how-to
manuals for something that can be listened to for entertainment, or reenacted as a haunted house. If I for
whatever reason get hurt while doing either, not only is that my responsibility, and hold no one else other
then myself responsible, including all entities and otherwise named above. Furthermore, all their directors,
officers, volunteers, representatives, and agents are NOT responsible for my negligence or carelessness in
staging such a risk-free event.


The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and
waiver to the maximum extent permissible under applicable law.


Piracy: This is intended for private use only. It is intended for you to stage this safely amongst family
members and close friends who you have been quarantining with. This is not for resale, or an opportunity
for you to sell tickets or make any money whatsoever.
